Interdisciplinary Management of a Patient with Dentinogenesis Imperfecta Type II

Now online in the Journal of Prosthodontics, a new clinical report on the management of a patient with dentinogenesis imperfecta type II, co-authored by ACP member, Francisco Azpiazu DDS MS.

Type II dentinogenesis imperfecta is an autosomal dominant genetic condition that affects dentin which increases the complexity of the predictability of restorative treatment. The clinical and radiographic features of this disorder include: teeth with bulbous crowns with pronounced cervical constriction, amber or opalescent brown shade, early loss of enamel resulting in accelerated attrition, obliterated pulp chambers, and rudimentary roots.

This clinical report describes the 2-year-long interdisciplinary treatment of a 20-year-old male with dentinogenesis imperfecta type II treated using a combination of CAD-CAM and analog techniques. A treatment plan involving maxillary and mandibular extractions, complete dentures, dental implants, and maxillary and mandibular implant-supported full-arch prostheses, and orthognathic surgery was proposed and accepted by the patient.

The clinical team used a combination of analog and CAD-CAM technologies to fabricate devices that aided planning, assisted intermaxillary fixation and implant placement, served as interim prostheses, and permitted the accurate establishment of esthetics and occlusion of the definitive full-arch prostheses.

This report demonstrates how CAD-CAM technologies can be used to create highly accurate devices and dental prostheses that simplify the planning and execution of advanced implant surgery and full-mouth rehabilitation.
