Dental Management of Obstructive Sleep Apnea in a Prosthodontic Practice

This presentation will provide an overview of how the prosthodontist can participate in providing treatment for patients who have been diagnosed with either snoring or obstructive sleep apnea by using oral appliance therapy. This will be a review for clinicians who are already providing this service and will also serve as a comprehensive overview of how to initiate this service. For the prosthodontist who is familiar with the oral pharyngeal region, this initiation of service is easily carried out with the fabrication of the appliance. It is necessary to learn the new terminology, how to effectively collaborate with the sleep physician, how to interpret the diagnoses and how to follow the proposed protocol.
    
Participating in this new area includes the following: (1) providing screening for general prosthodontic patients and head and neck cancer patients who have a high probability of obstructive sleep apnea after cancer treatment and (2) fabricating an oral appliance for patients referred by the physician. Information regarding these two capacities will be provided in this presentation.
Participating in this area of clinical practice offers several advantages. Most of the information collection, screening and clinical procedures can be performed by auxiliary staff who have been trained. Further advantages of providing this service include increasing the quality of life of patients and/or their bed partners and generating income that is usually paid by medical insurance if patients are diagnosed with obstructive sleep apnea.

Prosthodontic Management of the Patient with a Congenital Craniofacial Condition

Prosthodontic rehabilitation of congenital craniofacial patients requires a multidisciplinary approach. These patients require long term care from infancy until early adulthood with the Prosthodontists playing an integral role in the rehabilitation process. This presentation will highlight the prosthodontist’s role in the care of these patients and will present the current state of prosthodontic treatment for this patient population. Functional, physical, and emotional deficiencies of this patient population will be addressed.
At the conclusion of this session, the attendee should be able to:

  • Describe the role of the prosthodontist in the rehabilitation of congenital craniofacial patient.
  • Review the most common congenital craniofacial disorders which require prosthodontic rehabilitation.
  • Recognize the physical and functional dental deficiencies with this patient population.

Functionally Designed and Prosthetically-Driven Mandibular Reconstruction

Historically, the patient suffering from the resection of a mandibular tumor or destructive lesion would rarely undergo reconstruction and would be even less likely to have a functional prosthetic reconstruction as well. As the planning and surgical techniques associated with mandibular reconstruction have progressed significantly over the last 20 years, so too as the ability of the maxillofacial prosthodontist to provide a functional prosthetic reconstruction of the neo-mandible. Working closely in an interdisciplinary team setting, mandibular reconstruction can now be functionally and prosthetically designed and executed to achieve a new level of esthetic and functional outcome. The team approach to mandibular reconstruction at the Institute of Reconstructive Plastic Surgery at NYU-Langonge Medical Center will be presented as well as a historical perspective on the reconstruction of the patient with a mandibular resection will be reviewed.

  • Review the history of mandibular reconstruction techniques.
  • Describe the current state of mandibular reconstruction from prosthetic and surgical perspectives.

This presentation will describe the clinical steps involved in the execution of predictable surgical procedures in implant dentistry performed by prosthodontists as well as the outcome analysis of advanced and basic surgical procedures in implant dentistry.

Short Dental Implants an Alternative to Bone Grafting Procedures

Bone height is not always available to place a dental implant of 10-12mm of length and bone grafting procedures are not always an option. Additionally, grafting is a burdensome by adding cost and time to the treatment of partially or completely edentulous patients. Short dental implants can provide many benefits towards both the esthetics and function of the treatment outcome for these patients. This presentation will review different treatment alternatives with the use of short dental implants. Complications and solutions will be addressed.
At the conclusion of the session, the attendee should be able to:
  • Discuss the use of short dental implants as a treatment option, and understand the importance of diagnosis and treatment planning.
  • Identify the advantages of using shorter dental implants when resorting partially or completely edentulous patients.

Los maxilares no siempre tienen suficiente hueso para la colocación de un implante dental de 10-12mm de longitud y procedimientos de injerto óseo no son siempre una opción. Asimismo, los injertos incrementan el costo y la duración del tratamiento en pacientes parcial o totalmente edéntulos. Los implantes dentales cortos proporcionan muchos beneficios tanto para el paciente como para el especialista, nos brindan la posibilidad de evitar estructuras anatómicas vitales en casos que anteriormente se habrían necesitado tratamientos adicionales y nos pueden ofrecen beneficios en el área estética y funcional. Esta presentación examinará las diferentes alternativas, complicaciones y soluciones en el de tratamiento con implantes dentales cortos.

Implant Abutment Selection Using CAD/CAM Technology for Highly Esthetic Demands

This lecture will begin with a description of the implant abutments including impression abutments and prosthetic abutments. The advantages and disadvantages of each type of abutment and its specific clinical indications will then be emphasized as well as the step-by-step use of the implant abutments. The description and analysis of what is called the Zirconia monoblock system which I believe is the ideal system for implants supported restorations for the highly esthetic demand of today's dentistry will also be presented. Finally, a classification of the implant abutments will be discussed based on different clinical situations in current implant dentistry.

  • Describe current implant abutment selection according to patient situation.
  • Analyze implant abutment design depending on esthetic requirements and clinical indications.
  • Show current all ceramic implant abutments used for the anterior and posterior implants supported restorations.

  • Compare implant screw retained abutments vs. cement retained implant supported restorations.
  • Propose a simple and practical implant abutment classification according to patient needs, esthetic demands, restoration type, and clinical predictability.

 
Selección de los Pilares Protesicos Utilizando Tecnología CAD/CAM Para Satisfacer las Demandas Esteticas de la Implantologia Actual

Se iniciará con una descripción de los pilares protésicos sobre implantes incluyendo tanto pilares de impresión como pilares para retención de coronas. Se hará énfasis en las ventajas y desventajas de cada uno de los tipos de pilares y en sus indicaciones clínicas específicas. A través de la conferencia se describirá paso a paso del uso de los pilares implantológicos. También se hará una descripción y análisis de lo que hemos llamado el sistema monoblock en zirconia como un sistema ideal para restauraciones implantosoportadas para la alta demanda de la odontología estética de hoy. Finalmente, Se propondrá una clasificación de los Pilares Protesicos basada en las diferentes situaciones clínicas que se pueden presentar hoy en día.

Paradigm Shift in the Biomechanical Rehabilitation of the Dental Cripple: From  Overdentures to Fixed Prostheses

This lecture will provide an in-depth analysis of viable and cost-effective methods for the restoration of resorbed arches or arches with hopeless dentitions using dental implants and immediate loading concepts. The lecture will present surgical and restorative techniques that do not require the use of grafts or prolonged healing times with removable prostheses using axially placed or angulated dental implants (immediately placed in the case of dentate case), with immediate occlusal loading with an acrylic interim fixed prosthesis. The restoration process with the use of CAD/CAM generated framework and hybrid prostheses will also be described.
At the conclusion if this session, attendees should be able to:
  • Understand the value of immediately loaded fixed prostheses.
  • Understand the value of angled dental implants.
  • Review application for angled implants in compromised situations.
  • Understand the long-term benefits of this treament alternative.

Rehabilitación Biomecánica del Paciente Discapacitado Dental: Cambio de Paradigmas en Situaciones Complicadas


En esta conferencia se realiza un análisis en profundidad de posibilidades viables y costo-efectivas para maxilares edéntulos o maxilares con denticiones desahuciadas que permitan rehabilitación con satisfacción inmediata de los pacientes. Se presentaran técnicas quirúrgicas y restaurativas que evitan el uso de injertos o periodos prolongados de cicatrización con la inserción de implantes (inmediatos en casos de denticiones desahuciadas), todos carga oclusal transoperatoria mediante una prótesis transicional acrílica. La restauración de los casos utilizando técnicas CAD-CAM para la fabricación de las estructuras y prótesis hibridas o metal-cerámicas se describirá en detalle. 
 

 

Al finalizar esta sesión los participantes estarán en la capacidad de:

  • Entender la importancia de la carga transoperatoria en prótesis fijas para pacientes y odontólogo.
  • Entender la importancia de los implantes en posición angulada.
